Duly completed OPHL application form
One valid ID
If employed: COE, one-month payslip, employment contract, proof of remittance, and ITR
If self-employed: Latest AFS, business registration papers, 6-month bank statements, and ITR
Contract to Sell (for property purchase)
Building plan, specifications, and bill of materials (for construction loans)
Proof of expenses (for reimbursement cases)
Statement of account (for refinancing)
Fire Insurance and Contractor’s All Risk Insurance
Mortgage Redemption Insurance equivalent to the loan amount
You are not required to submit a marriage contract or provide a Special Power of Attorney. This simplifies the process for Japan-based applicants who may not have local legal representation in the Philippines.
